OVH accepts a wide range of insurance plans, including Medicare and Medicaid. If you have specific questions, contact your insurance provider directly using the phone number on your card. 

Our front desk team can help you apply. You will need to provide proof of income and household size. Please fill out the application for the Sliding Fee Scale. Any of the following documents can be used for proof of income: 

• Federal IRS Form 1040 

• Three (3) most recent pay stubs 

• Child support 

• Unemployment benefits 

• Social security benefits 

• Other proof of income 

• Zero Income Attestation Form 

We cannot accept bank statements as proof of income.
