OVH accepts a wide range of insurance plans, including Medicare and Medicaid. If you have specific questions, contact your insurance provider directly using the phone number on your card.
Our front desk team can help you apply. You will need to provide proof of income and household size. Please fill out the application for the Sliding Fee Scale. Any of the following documents can be used for proof of income:
- Federal IRS Form 1040
- Three (3) most recent pay stubs
- Child support
- Unemployment benefits
- Social security benefits
- Other proof of income
- Zero Income Attestation Form
We cannot accept bank statements as proof of income.

OVH is a nonprofit community health center that provides affordable, high-quality healthcare to individuals and families in our community, regardless of insurance or financial status. OVH is a recognized Federally Qualified Health Center through the Health Resources and Services Administration (HRSA).
An FQHC is a community-based healthcare provider that:
- Qualifies for funding under Section 330 of the Public Health Service Act (PHS)
- Qualifies for enhanced reimbursement from Medicare and Medicaid, as well as other benefits
- Serve an underserved area or population
- Offer a sliding fee scale
- Provide comprehensive services (either on-site or by arrangement with another provider), including:
- Preventive health services
- Dental services
- Mental health and substance abuse services
- Transportation services necessary for adequate patient care
- Hospital and specialty care
- Have an ongoing quality assurance program
- Have a governing board of directors

Yes. Interpretation services are available for over 240 languages and ASL video interpretation. Some providers are bilingual in English and Spanish

Patient Portal & Medical Records
Patient Portal is service through AthenaHealth that allows you to:
- View test results
- Request medication refills
- Message your care team
- Manage appointments
Fill out the Medical Records Release Form. Once the form is completed, we will process it and send it to the requested provider.
You can access Patient Portal through our website. If you need help, our staff can assist with setup.
