Chronic Care Management Comes to Oconee Valley Healthcare

In September, Emily Rollin, RN, joined our medical team as our Chronic Care Management Coordinator. Emily will work on streamlining health care plans for patients with chronic health conditions.

About the Chronic Care Management program:

For patients with multiple chronic conditions, juggling doctor visits, medications, and managing these conditions can quickly become overwhelming. Chronic care management services help deliver coordinated care to patients that will improve their health, increase satisfaction with their care, and make care more person-centered. Some highlights of the services are:

  • At least 20 minutes a month of chronic care management services
  • Personalized assistance from a health care professional who will work with you to create your care plan
  • Coordination between your pharmacy, specialists, testing centers, hospitals, and more
  • Phone check-ins between visits
  • 24/7 emergency access to a health care professional
  • Assistance with setting and meeting your health goals

If you have Medicare and live with two or more chronic conditions, ask your provider about chronic care management services.

Power of ONE and Pink Out Day

Oconee Valley Healthcare and St. Mary’s Good Samaritan Hospital invite you to join us for this year’s Breast Cancer Awareness campaign! On Friday, October 16th, we invite you to join our Pink Out day. Wear pink to show your support in raising awareness for breast health and breast cancer!

Power of One is a partnership to raise awareness about breast cancer prevention and early detection. It only takes ONE person to make a difference! Breast cancer is very prevalent in Greene County and community support is needed to encourage annual well woman exams and yearly mammograms. As always, Power of One also provides no-cost mammograms to uninsured women in the community during October. Call 706-453-5005 and mention the Power Of One to schedule during the week of October 12-16th.

If you have any questions, please call Allison at 706-454-5183.

National Health Center Week 2020

Oconee Valley Healthcare will be celebrating National Health Center Week from August 9th-15th. This is a week that is dedicated to showcasing the effective and innovative health center model of care each year. This year’s theme is “Community Health Centers: Lighting the Way for Healthier Communities Today and in the Future”. Oconee Valley Healthcare is proud to be a Federally Qualified Health Center, and has a couple of events planned to shine our light in the community. However, our celebrations this year will be limited due to the ongoing COVID-19 pandemic. As we celebrate the impact Health Centers have on their communities, let’s shine a light that will embody the future of primary health care access for underserved populations.

Our Milledgeville office moved to a new location right when the pandemic started to closing everything down, so we are celebrating now! On Wednesday, August 12th, we will be hosting a ribbon cutting ceremony to mark the opening of the new office!

As essential workers, our staff has worked tirelessly to adjust to the new normal and continue offering care to our patients through the duration of the pandemic. To honor all of their hard work, we will have a staff appreciation day on Friday, August 14th.

We hope that we can celebrate with the community again next year, and we appreciate the continued support!
